Experts warn that vocal cord paralysis (VCP) may occur if there is improper management of diabetes. VCP occurs when the vocal cords do not open or close properly. This would lead to a breathy voice or even a weak cough.

However, VCP is known to be life threatening with cases of choking also being identified. VCP also has various other symptoms such as hoarseness, vocal fatigue; mild to very severe lowering of vocal volume, shortness of breath, pain present in the throat when speaking, food or liquids travelling down the trachea (also known as aspiration), and resultant coughing that is recurrent.

Dr. Manal Ahmed Bukhari, who is a Phonosurgeon Consultant at the King Abdulaziz University Hospital, Saudi Arabia, stated how while there are many causes of VCP such as the vocal cord nerve being compressed from intubation, neck trauma, tumors or even surgical injury to the vocal cord, the link between diabetes and VCP have become a really significant concern. Dr. Bukhari further stated that Saudi Arabia has 26% of the population affected by diabetes while diabetic neuropathy (DNP) affects 50% of those diabetic patients. He added, “DNP may affect the cranial nerves, the vagus or even the recurrent laryngeal nerve, which can lead to unilateral or bilateral vocal cord paralysis.”

In order to stave off VCP, the main focus should be the proper management of diabetes through a healthy diet and a proper exercise regime. Doctors suggest a diet that consists of mainly fruits, vegetables and legumes along with the absence of processed foods and sugar is ideal for diabetics.

While it has also been widely accepted that an exercise regime that is cardiovascular in nature should be considered and practiced consistently, Dr. Bukhari also believes that examinations such as the pre-operative endoscopic examination may detect any paralysis or paresis.

According to a survey conducted by the Diabetes Foundation of India and the National Diabetes, Obesity and Cholesterol Foundation (N-DOC) of New Delhi, the majority of kids in the country’s capital are obese. The survey indicated that 13% of these students were overweight or obese due to the consumption of unhealthy food.

The survey focused on 25 schools between 2007 and 2010 that were either private of funded by the government. Exactly 4,704 children in Jaipur were covered, and 2,380 were from private schools. The chief project officer of the Diabetes Foundation, Priyali Shah stated, “Obesity among school kids make them prone to diabetes, hypertension and stress in adulthood. It is caused by unbalanced diet and sedentary lifestyle of urban children.”

Students who were within the age of 14-17 were found to display a high prevalence of being either overweight or obese. The reason for this prevalence could be due to school hours, tuition time and also preparation for competitions. As more time is spent away from home and children having more freedom and pocket money, the focus is on food that is available in school canteens. The usual fare would include samosas, noodles, and burgers amongst other junk food. Dr Sailesh Lodha, a city-based endocrinologist believes that this rate of 13% is not that high when compared to other cities such as Allahabad, Agra, Delhi, Dehradun and Mumbai.

Lodha’s survey on 500 school kids across 8 schools, known as ‘Creating awareness on Obesity and Diabetes’, noted that the root cause for obesity is a sedentary lifestyle plus a lack of physical exercise.

The survey also found out that junk food, which is offered in most school canteens were exacerbating the situation for children. The majority of such junk food was less expensive thereby making it more accessible. Such food also required improper storage, making it less costly for schools.

Scientists from the Center for Nutritional Genomics at the Salk Institute for Biological Studies have found out how insulin increases fat storage during feeding while another study led to the breakthrough of a new “fasting pathway”, which points the route to a set of diabetes drugs that can bring back unusually high glucose levels in blood to normal.

William R. Brody, M.D., Ph.D., president of the Salk Institute, believes that these types of discoveries can be helpful when it comes to improving the health conditions of diabetics.

The studies also revealed that a set of enzymes known as histone deacetylases or HDACs, play a role in controlling the energy balance of the body by stimulating sugar production when glucose levels are low due to fasting. There needs to be balanced between excessive storage of glucose in fat tissue and muscle and the production of glucose in the liver.

If this balance is not strictly regulated then glucose levels in the blood can fluctuate leading to damage in tissues and cells. Marc R. Montminy, M.D., Ph.D., professor in the Clayton Foundation Laboratories for Peptide Biology at Salk, likens the metabolic system of our bodies to a hybrid car, “In the daytime we use glucose as high-octane fuel, but at night we switch to the ‘battery,’ which in this case is stored fat.”

However, in diabetics the liver produces glucose even when glucose levels are high. Using mice with diabetes, the Salk researchers found that the suppression of HDACs led to the normalizing of blood glucose levels. Reuben J. Shaw, Ph.D., a Howard Hughes Medical Institute scientist and an assistant professor in the Molecular and Cell Biology Laboratory at Salk, believes that this suppression of HDACs via certain drugs could be used any potential drugs for those suffering from diabetes.

British researchers have found that diabetes and cholesterol have a gene that controls genes that are found in fat within the human body. The researchers believe that fat plays a vital role in the susceptibility to obesity, diabetes and heart disease, which are collectively known as metabolic disorders.

The identification of this regulating gene could help drugs target the gene specifically when treating such illnesses. According to Tim Spector of King’s College London, the chief author of the study, this new research can be considered as a major study and the first of its kind, which shows how minute changes in one master gene can lead to metabolic effects in various other genes.

A gene known as KLF14, that is linked to Type 2 diabetes and cholesterol, has already been identified by scientists. However, it is only now that they have found out the exact role it plays. Spector and his team analyzed over 20,000 gene samples from fat, taken from 800 female twins. A link between KLF14 and other genes in fat tissue have been identified highlighting the fact that KLF14 plays the role of a “master switch” in controlling these genes. These findings were compared against 600 fat samples from volunteers in Iceland.

In a report, the researchers stated that KLF14 is linked to metabolic traits such as cholesterol, obesity, body mass index, glucose levels, and insulin. Mark McCarthy from Britain’s Oxford University, who was also one of the researchers on the study, stated that KLF14 acts as a controller when it comes to “processes that connect changes in the behavior of subcutaneous fat to disturbances in muscle and liver that contribute to diabetes and other conditions”.

As McCarthy went onto say, the main focus now should be how this research can help improve the treatment of such metabolism-related conditions.

According to research from the University of Alabama at Birmingham, even a minute difference in diet could affect diabetes risk. In this particular study, which was funded by the National Institute of Diabetes and Digestive and Kidney Disease, there were 69 healthy but overweight candidates who were not afflicted by diabetes but were at risk of developing it, were placed on specific diets such as low-carb diets.

The diets included reductions in fat or carbs for a total of 8 weeks. Barbara Gower, Ph.D., a professor in the Department of Nutrition Sciences at UAB and lead author of the study stated that “At eight weeks, the group on the lower fat diet had significantly higher insulin secretion and better glucose tolerance and tended to have higher insulin sensitivity.”

What is unique about this study is that weight loss was not taken into account. Rather the focus was on diet quality and not diet quantity. The participants involved in the study were fed the same amount of food necessary to retain their body weight.

The researchers considered small fluctuations in body weight during the analyses. The results indicated that reducing the daily consumption of fat to approximately 27% could help minimize their risk of diabetes over the long term. These findings were more relevant when it came to African-Americans.

African Americans who were on the diet that was low in fat indicated a large difference in insulin secretion when compared to those who were on low carbs. The participants in the study were on a low-fat diet that consisted of 27% fat and 55% carbs whereas; the participants on the low-carb diets were on 39% fat and 43% carbs. The results of this study are certainly significant, as per Gower, since there are certain groups who are at a higher risk of developing diabetes such as Native Americans, African-Americans, Latinos and the elderly.

According to researchers, a certain cholesterol lowering drug that is taken in combination with an anti-depressant, taken by over one million people in America, could lead to a spike in blood sugar levels.

Researchers at the Stanford University School of Medicine, Harvard Medical School, and Vanderbilt University believe that the anti-cholesterol drug Pravachol and the anti-depressant Paxil do not have the effect if taken separately. This interaction was discovered by assessing reports of adverse events present in a US Food and Drug Administration database, and comparing them to electronic medical records of the aforementioned medical institutions.

The researchers established that an average increase of 19 milligrams per deciliter in blood glucose were present after initiating the treatment in 135 non-diabetic patients. However, 14 people with diabetes who started the treatment experienced a greater increase of 48 mg/dl. A spike in blood sugar is enough to push a person who is pre-diabetic into full blown diabetes.

This assumption was put to the test with lab mice that were fed a high-calorie diet that was also high in fat. This put them in a state that was pre-diabetic and insulin resistant. These pre-diabetic mice had their blood glucose levels soar from 128 mg/dl to 193 mg/dl when they were put on the two drugs for three weeks.

According to Russ Altman, professor of medicine at Stanford believes that drug interactions of this nature occur most of the time in the human body. However, since “they are not part of the approval process by the Food and Drug Administration, we can only learn about them after the drugs are on the market.” Altman believes that there are between 500,000 and one million people taking this combo simultaneously. Patients should understand this situation and its effects on blood sugar, which would allow a diabetic to better understand his or her condition. Importantly it could also prevent a person from reaching a state of full blown diabetes as well.

Researchers have reported in two different studies that the genes, which control lipid levels, could lead to an early prediction of diabetes as well as heart disease and atherosclerosis. Joanne E. Curran, PhD, of the Texas Biomedical Research Institute in San Antonio, and colleagues found that in one study, the lipid genes, known as dihydroceramide (dhCer) coded by a region on chromosome 3 seemed to be a diabetes risk factor irrespective of what the insulin and glucose levels were.

This was reported by Sara Willems, PhD, of Erasmus Medical Center in Rotterdam, the Netherlands, and her colleagues.

Willems stated, “As our knowledge of genetic variation increases, preclinical genetic screening tools might enhance the prediction and prevention of clinical events.” Donna Arnett, PhD, MSPH, of the University of Alabama at Birmingham School of Public Health, and a spokesperson for the American Heart Association believes that conventional clinical risk factors can lead to diabetes and heart disease, however, adding genetic tools to this mix could be helpful especially in ascertaining how to actually approach prevention in patients who are considered to be at risk by conventional measures.

The group headed by Willems utilized the Erasmus Rucphen Family Study and the Rotterdam Study, which is population-based, to assess top lipid-associated single nucleotide polymorphisms (SNPs) that have been identified by a genome-wide meta-analysis recently.

The studies took into account 10,212 individuals along with measurements of plaques and carotid intima media thickness (IMT). An analysis of one million SNPs in quantitative and genome-wide association analyses showed that lipid genes were inherited. There are 128 lipid species, which has the ability to predict diabetes incidence over a decade of follow-up. As per the study, dihydroceramide 18:0 is the ideal predictor of progression to diabetes and was significantly heritable and more common amongst diabetics.

Currently the number of children in Australia who suffer from Type 1 diabetes is high when compared to global levels. The Australian Institute of Health and Welfare believes that there will be a 10 percent increase by 2013. This revelation comes amid calls for a kidney screening program to identify early signs of kidney disease that has been triggered by the onset of Type 2 diabetes.

According to AIHW spokeswoman Anne-Marie Waters, “The rates we are predicting will actually rise from about 140 cases per 100,000 children to about 153 cases per 100,000 children.”By 2013, the rate of prevalence is expected to increase sharply by 10%. While an exact reason for this increase cannot be identified, it has been suggested that environmental changes have driven the increase in Type 1 diabetes.

Other aspects that may lead to this increase could be linked to diet, vitamin D deficiency, viruses, and also early exposure to cow’s milk. But with the increase of kidney patients who suffer from diabetes on the rise compared to those who are being affected by Type 1 diabetes, there are more calls for a program.

Kidney Australia medical director Dr Tim Mathew states that dialysis patients who suffer from diabetes have increased to 44 percent across Australian within the last 25 years. Kidney Australia had commissioned a report from Deloitte Access Economics to evaluate the financial effectiveness of a screening program for kidney disease.

As Mathew states, early detection is a priority in order to ensure that patients can control their condition thereafter. The best cure is to ensure there is prevention. As per Mathew, this all depends on one’s lifestyle, which is a solution that is more long term. The short term goal at this point, however, is to screen people to see whether they are suffering from kidney disease.

According to the Canadian Agency for Drugs in Technology and Health, diabetic people can get anxiety and depression if they used to checking their blood sugar level up to 10 times a day.

One of the most convenient methods that were used to keep a tab on blood sugar happens to be an early innovation. It was during the 1980s that the first diabetic testing supplies, blood glucose meter, were marketed. While such a tool had been beneficial to the 180 million plus diabetics around the world, the question remains – “Is this modern practice of consistent blood-glucose-checking safe?” As it turns out, such a practice can be potentially dangerous according to the Canadian Agency for Drugs in Technology and Health. Moreover, it could even lead to anxiety and depression.

Diabetics Testing Supplies: Blood Glucose Meter

There are those who have an unhealthy fascination with checking their blood sugar level and may repeat the procedure around 10 times per day. While depression and anxiety alone are negative factors, there is financial aspect to consider as well. Each of these strips cost around a dollar. Needless to say, this could lead to the expenditure of thousands of dollars per year. For instance, it has been surmised that $3 million per year could have been saved by Nova Scotians and then invested in prevention programs. Yet when public health agencies create a culture of fear, the public responds accordingly.

Instead of pursuing such a practice, what diabetic people need to pursue is a lifestyle suitable for diabetics as per most medical practitioners. Once this lifestyle has become a habit, there should be a stable blood glucose levels. For instance, it has been touted time and time again that an active lifestyle along with a proper diet will help diabetics led a normal life. Choosing a diet that is rich in fiber with a focus on fruits and vegetables has proved to stabilize blood glucose levels. Additionally, some form of light exercise, walking or even golf can further help regulate the levels of blood sugar.
