Diabetic Pregnancies Raises Risk of Diabetes

The most worrying issue here is that gestational diabetes may at some point lead to Type 2 diabetes later on in life, as per research. According to Dr. Brian Egan, Provena Saint Joseph obstetrician/gynecologist, almost half of the women who are afflicted with gestational diabetes end up developing Type 2 diabetes within just five years of their pregnancy. In comparison, 70% of these women develop diabetes within just 28 years after their pregnancy. Egan went onto state, “Pregnancy tends to be a little crystal ball of the future. It tells us what the future could be, but we do have the opportunity to change that future.”

Egan also believes that pre-eclampsia could lead to cardiovascular diseases later on in life. However, on the bright side, there are just 8% of pregnancies that proceed onto becoming gestational diabetes cases. Yet these individuals have a higher chance of developing Type 2 diabetes much later. Egan reiterated that the risk of birth trauma is relatively higher for mother and baby when the mother is afflicted with gestational diabetes, since the babies are usually larger.

Additionally, it has been found out that if the mother has vascular problems, then there is a higher chance of the baby being born smaller or with malformations caused by problems within the placenta. Mothers who do have gestational diabetes may have their children become hypoglycemic right after delivery, with insulin levels that are high. At this juncture, both the mother and baby could be at a higher risk of being affected by diabetes.

With gestational diabetes affecting 135,000 women every year, which is close to 5% of pregnant women, Egan recommends that women consider medical checkups before they do decide to become pregnant.

William R. Brody, M.D., Ph.D., president of the Salk Institute, believes that these types of discoveries can be helpful when it comes to improving the health conditions of diabetics.

The studies also revealed that a set of enzymes known as histone deacetylases or HDACs, play a role in controlling the energy balance of the body by stimulating sugar production when glucose levels are low due to fasting. There needs to be balanced between excessive storage of glucose in fat tissue and muscle and the production of glucose in the liver.

If this balance is not strictly regulated then glucose levels in the blood can fluctuate leading to damage in tissues and cells. Marc R. Montminy, M.D., Ph.D., professor in the Clayton Foundation Laboratories for Peptide Biology at Salk, likens the metabolic system of our bodies to a hybrid car, “In the daytime we use glucose as high-octane fuel, but at night we switch to the ‘battery,’ which in this case is stored fat.”

However, in diabetics the liver produces glucose even when glucose levels are high. Using mice with diabetes, the Salk researchers found that the suppression of HDACs led to the normalizing of blood glucose levels. Reuben J. Shaw, Ph.D., a Howard Hughes Medical Institute scientist and an assistant professor in the Molecular and Cell Biology Laboratory at Salk, believes that this suppression of HDACs via certain drugs could be used any potential drugs for those suffering from diabetes.

None of these kids had Type 2 diabetes before the treatment. Additionally, there were 26 kids whose bodies were unable to process blood sugar effectively. This is a well known precursor to developing Type 2 diabetes.

The children in the study underwent growth hormone therapy for a variety of conditions such as short stature, human growth hormone deficiency or genetic disorders, for example Turner syndrome or Prader-Willi syndrome. All those who participated in treatment took Humatrope, which is marketed by Eli Lilly. As per the National Institutes of Health, there are 8 out of every 100,000 kids that are aged between 10 to 19 years who are diagnosed with Type 2 diabetes annually. For the sake of comparison, the rate seen in the kids on growth hormone represents 100 out of every 100,000.

Dr. Christopher Child, the lead author of the study and a researcher at Lilly Research Center in England, reiterates that human growth hormone does, in fact, interfere with the activity of insulin but it is unclear whether this hormone leads to an increase in the risk of diabetes in kids.

Child also recommended the “closer surveillance of (growth hormone)-treated patients with pre-existing type 2 diabetes risk factors before, during and after treatment.” This is a route that needs to be considered in order to assess glucose metabolism plus to promote lifestyle measures such as diet and exercise, which are known to effectively prevent diabetes.

By December 2010, according to Bernadette King, of GlaxoSmithKline, the manufacturer of the drug, there were approximately 90,000 people who were taking an Avandia-based product. Back in 2007, Nielsen published a research report that showed that Avandia increased the risk of heart disease by 40% in those suffering from Type 2 diabetes. Other diabetic drugs such as Actos and Metformin have not been proven to be this risky for diabetic users.

The Federal Drug Administration of the US (FDA) has made the decision to limit the use of the drug to those patients who have been treated with it without any side effects, or those whose blood glucose levels cannot be treated or controlled with other medications.

During June 2010, over half the FDA advisory committee suggested banning Avandia from the market or restricting access to it. These restrictions will now be enforced in November. These restrictions will also be applicable to Avandaryl (which is a combination of both rosiglitazone and glimepiride) and Avandamet (which consists of rosiglitazone and metformin).

According to Harlan Krumholz, a Yale cardiologist and longtime critic of the drug, these recent restrictions have been a long time coming especially since there is ample evidence to suggest that patients who are on Avandia are at real risk of heart disease. It was during 2007 that David Graham, a FDA drug-safety expert reported that Avandia not only caused heart problems but also the deaths of close to 200,000 people.

The HealthPartners Diabetes Wizard, which builds on strong results that helped patients achieve control of their diabetes, will modify and personalize a treatment plan for every single patient based on medical evidence, including medications that are recommended for better blood sugar, blood pressure, and cholesterol levels.

According to Beth Averbeck, MD, Associate Medical Director for primary care of HealthPartners Medical Group, an estimated one-third of Medicare costs come from diabetes or diabetes-related complications, including heart disease and stroke, so there’s no question that new tools are needed to better manage the disease.

The Wizard is a tool that takes into consideration more details from patients compared to what was previously presented in terms of tracking the medical records of the diabetics. These included information on kidney function, patient age, smoking status, current medications, and history of congestive heart failure. Additionally, the Wizard can also indicate if there have been gaps in care, such as the requirement for a screening test for kidney disease.

The Diabetes Wizard was primarily developed by the HealthPartners Research Foundation in conjunction with the HealthPartners Medical Group. This tool was researched from October 2006 to May 2007 where 2,500 patients with Type 2 diabetes, 40 primary care physicians, and 11 clinics were part of the study.

According to Tracie Patten, Nevada market manager for the American Diabetes Association, “Simply implementing a healthy diet and exercise plan can prevent and reverse Type Two diabetes in most cases”. She reiterated that the prevention of Type 2 diabetes is a reality that needs a proper course of action.

As per the American Diabetes Association, both diabetes and obesity are very much related. Eating healthily should be the prime focus for both children and adults so that obesity and Type Two diabetes along with its complications can be easily curbed. At present diabetes has affected close to 26 million children and adults with 79 million being the victim of pre-diabetes. It has been estimated by WHO that the number of Americans with diabetes is set to increase by a large margin to over 30 million by 2030.

However, it is worthwhile noting that the spray cannot be used as a solution for those suffering from diabetes since the discovery is only just a step from becoming an actual vaccine for diabetes. In addition to helping combat diabetes, the spray could also combat other auto-immune diseases like rheumatoid arthritis and multiple sclerosis.

There are three types of diabetes that are known to man. One is known as gestational diabetes that affects those who are pregnant while the most common form is known as Type 2 diabetes. Type 1 diabetes leads the body to destroy insulin producing cells found in the pancreas. When there is a lack of insulin, the body helps create an increase in blood sugar and urine glucose. This is a condition that can be fatal unless sufferers consider getting daily injections of insulin. The nasal spray vaccine could prevent Type 1 diabetes from increasing in at those individuals who are at risk.

Researchers in Melbourne have tested the spray in young people with a family history of Type 1 diabetes. Walter and Eliza Hall Institute‘s Professor Len Harrison stated ”This is the first time anyone has shown that this novel vaccine approach can change the immune response.”

This research could offer up a vaccine for over 150,000 Australians who suffer from diabetes. According to a report from the Institute of Health and Welfare, there is a rise in diabetes in mainly the young. This vaccine can curb that rise. Form a financial standpoint, the Australian health system can save $600 million annually from diabetes costs.

This decision was taken after a government-funded study proved that the drugs led to an increased risk of bladder cancer. The agency informed doctors in France to stop all prescriptions of such drugs to patients. However, people who are currently using the drugs consult their doctors. After reviewing the French report, which assessed diabetics from 2006 to 2009, German health officials also followed suit.

Robert Spanheimer, Takeda’s vice president of medical and scientific affairs, stated that “The FDA has had a look at all our data and have not given us an indication at this time.” While Takeda did not review the French study, two ongoing American trials has yet to show bladder cancer risk. This includes a 10-year trial that was started in 2003.

Actos has been reviewed by the FDA since September 2010. This review took place after diabetics who took the drug for over two years faced an increased risk for developing bladder cancer. According to WebMD, during that period the drug was considered much safer compared to Avandia. Actos, which is also known as pioglitazone, prepares the body more sensitive to the human hormone insulin, which is used to control blood glucose levels.

It’s patients who suffer from type 2 diabetes that are prescribed the drug.
